    Well i'd suggest a different order to the questions at least. For example. Questions number 3 is an opinion about the company image that you get from an ad, but you haven't found out yet whether they even heard about your company from an ad. Maybe they heard from word of mouth or a friend (which should be an option for Question 4 btw) I would move question #3 down the list closer to 9 or 10 with the other opinon questions. Also maybe add a question for them to suggest what could be done to make the ads appeal to them more.
